- Order ABG 15-20 minutes after ventilator setup to verify adequate gas exchange
- Adjust RR to target EtCO2 35-40 mmHg (PaCO2 may be 2-5 higher than EtCO2)
- Set ventilator alarms: high pressure, low tidal volume, disconnect, apnea
- Consider recruitment maneuver after intubation if SpO2 <95% on FiO2 >0.5
